Introduction

We are seeking a skilled QI/QA Analyst to join our team and aid in the reviewing of our services to meet our high quality standards here at Seldovia Village Tribe. As we strive for excellence, we require a competent QI/QA Analyst with previous experience who can maintain and enhance our already exceptional reputation through careful patient outcome evaluation. The ideal candidate will possess top\-notch organizational skills and a methodical approach, as well as the necessary expertise to elevate our patient and community care to new heights in terms of precision, security, and safety. This individual will play a vital role in establishing a comprehensive program that not only guarantees patient quality care, but also fosters a culture of active employee involvement. You will collaborate with a small team to ensure that our services meet the high quality standards of our organization.

Position Overview

The Quality Improvement/Quality Assurance (QI/QA) Data Analyst functions as a critical part of our quality and clinical applications team, performing data management, analysis and reporting functions to support our organizations participation in, and compliance with, HRSA QI/QA and NCQA PCMH requirements.
Process Improvement:
Working closely with the Medical Director, act as the QA/QI leader responsible for all Continuous Quality Improvement Committee (CQIC) meetings, tracking improvement activities, and facilitating change processes.
Reporting:
Utilizing various reporting systems and spreadsheet tools, mine data appropriately to provide accurate and verified data needed for clinical and operational process improvements. Work directly with individual departments to identify analytical requirements and provide needed data upon request. Responsible for internal and mandated external demographic and quality reporting for the health center.
Accreditation and Recognition:
Provide ongoing guidance to clinic leadership on alignment of operational processes and work flows with Patient Centered Medical Home(PCMH) standards. Pursue PCMH Certified Content Expert Status, and serve as the Transformation Manager of the program. Support clinic leadership in other accreditation efforts.

What You'll Do:

Assist with HRSA initiatives.
Process Mapping: Assist in overall process development and design for SVT Health \& Wellness services.
Coordinate initiatives and lead implementation of PCMH model for Community Health Centers and progress evaluation tools. Work with management and staff to facilitate change using a PDSA cycle analysis.
Act as Quality Assurance/Quality Improvement leader responsible for all Continuous Quality Improvement Committee (CQIC) meetings and activities, minutes and facilitating change processes for management of risks.
Actively assist with community engagement around data and quality data.
Program Development: 35%
Patient Centered Medical Home and accreditation support.
Day to day management of the 340B Program.
Continuous Quality Improvement Committee Co\-Chairperson.
Manage Progress, Evaluation Tools, PDSA of programs and projects.
Management of special projects, as assigned by the SVTHW Director.
Scheduled Reporting: 20%
UDS, MU, SDPI, MIPS, MACRA other Grant Reporting
Internal Reporting 35%:
Clinical Measures, Performance, Production, Financial
Policy and Procedure 10%:
Create Draft Policy to meet Compliance, recognition and accreditation standards,quality and organizational needs.
Stay abreast of regulations that affect the organizations policies and procedures.
Maintain a reporting manual to facilitate quick access to needed information. Interprets results using a variety of techniques ranging from simple data aggregation for complex data mining.

What You'll Need:

Bachelors degree preferred in business, health care management, social science, healthcare data analytics, or related. Minimum 2 years work experience in healthcare data analytics, quality improvement, risk management, and/or healthcare compliance.

Travel Requirements

Travel to between communities, Seldovia, Homer or Anchor Point by boat, plane, or car may be necessary.
